Life Insurance and Accidental Death and Dismemberment (AD&D)

  • $50,000 Employer-paid group term life insurance
  • Voluntary Life Insurance – Accident Insurance, Critical Illness Insurance, Hospitalization Indemnity Insurance and Prepaid Legal Insurance available
    • Available in increments of $10,000 up to the lesser of 5 times salary or $1,000,000
    • Guaranteed issue up to $400,000 (excludes $10,000 included with medical insurance)
    • Evidence of Insurability (EOI) above guarantee issue
    • Option to enroll in life insurance for spouse up to $100,000 and dependent child(ren) in increments of $10,000
    • Guaranteed issue: up to $50,000; any amount over that requires EOI. Cannot be more than 50% of employee’s coverage
    • Age Reduction: Reduced to 65% at age 65, 40% at age 70 and 25% at age 75
