Life Insurance and Accidental Death and Dismemberment (AD&D)
- $50,000 Employer-paid group term life insurance
- Voluntary Life Insurance – Accident Insurance, Critical Illness Insurance, Hospitalization Indemnity Insurance and Prepaid Legal Insurance available
- Available in increments of $10,000 up to the lesser of 5 times salary or $1,000,000
- Guaranteed issue up to $400,000 (excludes $10,000 included with medical insurance)
- Evidence of Insurability (EOI) above guarantee issue
- Option to enroll in life insurance for spouse up to $100,000 and dependent child(ren) in increments of $10,000
