Plan Options:

  • High Deductible Plan (HDP): Includes HSA with employer contribution
  • Copay Plan: Fixed office visit copays with moderate premium
  • Buy-Up PPO Plan: Lower deductible and out-of-pocket maximums

All plans include:

  • Preventive care covered at 100%
  • Mail-order pharmacy services
  • Access to Optum Rx for prescription drug management
  • Specialist visits without referrals
Routine care is covered with minimal cost-sharing through Delta Dental and DeltaVision.

Dental Benefits:

  • Preventive care at no cost
  • Coverage for basic, major, and orthodontic services

Vision Benefits:

  • $10 annual eye exam copay
  • $150 frame or contact lens allowance
  • National in-network provider access

We offer coverage that helps safeguard employees and their families in the event of illness, injury, or loss.

Coverage includes:

  • Employer-paid Basic Life and AD&D Insurance
  • Employer-paid Short- and Long-Term Disability
  • Voluntary Life and AD&D options
  • Critical Illness, Accident, and Hospital Indemnity plans (via Colonial Life)
We support tax-advantaged savings tools to help manage care-related expenses.

Available Accounts:

  • Health Savings Account (HSA): Includes employer contribution
  • Healthcare Flexible Spending Account (FSA)
  • Dependent Care FSA
  • Limited Purpose FSA for dental/vision (when paired with HSA)
  • $640 rollover for unused FSA funds
